One of the characters in 13 Sentinels: Aegis Rim, Tsukasa Okino, is non-binary, and says that "some binaries don't work for him". Though he uses he/him pronouns, he is often dressed in feminine clothing. One of the playable characters, Takatoshi Hijiyama, has a non-avoidable romantic subplot with Tsukasa, during which he develops a crush on Tsukasa when he thinks he is a woman, and then processes his feelings for him. It is implied that in the future, the two are in a long-term romantic relationship.

There are instances of transphobia in this game, as several characters make transphobic comments about Tsukasa for which there is no pushback. Some go as far as to call him a pervert when they see him in a dress, which is extremely offensive.
