EXTREME MEATPUNKS FOREVER includes a diverse cast of queer characters. The four protagonists are all queer. One is a gay man, another is a trans man, one is a lesbian, and the other dies not define their gender, using they/them pronouns but wishing to look more feminine. There are also several queer NPCs.

The game comes with a number of trigger warnings, including warnings for body horror, self harm mention, suicidal ideation, racially charged violence, alcohol, abuse mention, gender dysphoria, loss of bodily autonomy, apocalyptic ideation, and cults.
