One respondent to the Queer Representation (2016) survey praised Queen at Arms for its representation of gender and sexuality. The player-character is assigned female at birth and raised as a boy to hide their identity; halfway through the game, the player is able to consider the gender that the protagonist identifies with most, with man, woman, and genderqueer options available. The selection alters the way the protagonist interacts with other characters throughout the game, and makes an interesting and nuanced comment on gender identification.

The game also features variety between sexualities and types of relationships, with the protagonist not always marrying their love interest, and with epilogues being used to show the continuation of relationships so that the game does not 'finish' with marriage.
